Residential Group Homes

Congregate living arrangements with up to six (6) individuals. Services provided include skills training in activities of daily living, home management, community integration and 24-hour supervision.

Individualized Supported Living
Provides support and training services to an individual in his or her own residence. Individuals may live alone, with their family or share arrangements with others.

Day Habilitation
Services may be delivered in home or in the community, in a group or an individual, one-on-one basis. Groups include a ratio of one staff to four individuals. Training and support are intended to maximize self-determination and participation in the community and address a specific functional purpose.

Personal Assistance
Services include assistance with any activity of daily living (ADL) or instrumental activity of daily living (IADL). Assistance for ADLs include bathing, toileting, transfer and ambulation, meal preparation, feeding, incidental household cleaning and laundry. IADLs include shopping, banking, budgeting, using public transportation, social interaction and recreation and leisure activities.

Respite Services
Temporary care provided by trained, qualified personnel, on a regular or intermittent basis, with the purpose of meeting family needs or maintaining the physical and mental well-being of the client.
